Scope of the Role
The Registered Practical Nurse (RPN) reports to the Director of Care and plays a vital role in delivering high-quality, resident-centered care. The RPN provides leadership, supervision, and education to the healthcare team, ensuring residents’ physical, emotional, and psychosocial needs are met in accordance with professional standards and regulatory requirements.
Key Responsibilities
- Deliver direct nursing care and support to residents based on individualized care plans
- Administer medications and treatments as prescribed, ensuring safe and accurate documentation
- Monitor residents’ conditions and promptly report any changes to the care team or physician
- Develop and update care plans in collaboration with residents, families, and interdisciplinary teams
- Supervise and support PSWs and fellow RPNs, fostering teamwork and accountability
- Promote resident-first care in alignment with the Residents’ Bill of Rights
- Participate in assessments, admissions, transfers, and discharges
- Maintain accurate and timely clinical documentation
- Ensure compliance with LTC standards, infection control practices, and safety protocols
- Conduct shift rounds and ensure continuity of care between shifts
- Manage nursing supplies and equipment efficiently
- Address resident and family concerns with empathy and professionalism
- Contribute to quality improvement initiativesand staff education
Job Qualifications
- Registered Practical Nurse (RPN)in good standing with the College of Nurses of Ontario (CNO)
- Minimum 1 year of nursing experience(long-term care experience preferred)
- Current CPR certification
- Knowledge of RAI-MDS; AIS certification required or must be obtained within one year of hire
- Strong communication, leadership, and interpersonal skills
- Understanding of relevant LTC legislation, IPAC practices, and quality standards
- Compassionate and resident-focused approach to care
- French language skills considered an asset
